5 whys

This is a technique oriented at solving problems. 
The approach is to ask a series of why questions to understand the problem at hand and to find a solution to the basic cause.
Here is an example to demonstrate how:
Why didn't I make it in this exam?
A: Because I couldn't attempt all the questions.
Why couldn't I attend all the questions?
A: I was bit mentally prepared to face the exam.
Why wasn't I mentally prepared?
A: I hadn't faced enough exam like situations beforehand.
Why?
I hadn't written enough mock exams.

Ok, maybe every problem may not find a single answer to every why question that follows, but the technique is incredibly useful in locating the root  cause and finding a  solution.  This technique  was  apparently  developed by the Toyota management,  to deliver business solutions.
